Companion/Sitter Services

Companion sitters are individuals who furnish personal attendance, companionship, or household care services to children or to individuals who are elderly or disabled. Companion care is a form of home care offering non-medical services to older adults or people with disabilities. Companion care differs from personal care in that personal care aides (also known as home health aides) may provide the same services as companion care but also help with tasks such as bathing, dressing, and medication management, depending on specific state regulations. The goal of companion care is primarily emotional support and socialization.

Community Living and Support

Community Living and Supports (CLS) is a tailored program that aims to support people in effectively residing in their own homes, with family or other natural supports, and engaging in community life. A paraprofessional helps the client acquire new skills, put those they already have into practice, and provides the oversight they need to become more independent. The program seeks to improve self-sufficiency, foster community involvement, and improve life skills. With an emphasis on the individual’s specific preferences, CLS provides assistance in areas like daily living skills, nutrition, health monitoring, and community involvement. Enabling the recipient to live freely and completely integrate into their community is the ultimate objective.

Individual and Transitional Support

This hands-on service supports individuals with Serious and Persistent Mental Illness (SPMI) by teaching and assisting with daily living skills like meal preparation, medication management, shopping, and money management to help them live independently.

Individual and Transitional Support is a one-on-one service for those aged 16 and older with SED, SMI, SPMI, or severe substance use disorder (SUD). It provides structured interventions to enhance the beneficiary’s ability to manage daily living tasks, supporting independent living, recovery, and skill development in areas like employment, transportation, housing, finances, and education.
